There are a number of forms that need to be filled out in advance to satisfy the various rules/regulations imposed on our Pack by Scouting America, the State of Minnesota, and/or St. Joseph Catholic Church (our charter organization). You can find links to the forms below. In order to save paper, unless otherwise noted all these forms can be filled out electronically and submitted by emailing them to forms@sjpack207.org.
Scout Event Waivers
Fill out one of the these for each Scout and submit electronically by emailing forms@sjpack207.org. Filling out a waiver doesn’t obligate you to attend that event. Please submit electronically.

A photo release must be filled out for all youth participants and submitted electronically by emailing forms@sjpack207.org.
Parent "Talent" Survey
Each family should fill out the parent “talent survey available at the link below. Please do this even if you think you have no “talent”. After you finish it, please submit electronically by emailing forms@sjpack207.org.
Wreath Sales Aid
Please use the following Wreath Sales Sales Aid when doing wreath sales this year. All items and prices are included on the form. If you need additional order forms, please make extra copies of the last page.
Health Forms
Fill out AND PRINT one of these for each Scout and adult participant. A physical copy must be given to either the Cubmaster or your Den Leader. These will be retained in a secured folder that will accompany the Pack on all activities. Copies will be destroyed at the end of the year.

Allergies and Dietary Restrictions
If any member of your family that will be attending Scout activities has dietary restrictions, please fill out one of these forms and submit it electronically by submitting to forms@sjpack207.org.
Pack Shirt (Class B Uniform) Order Form
In addition to the official Cub Scout uniform, our Pack also uses a “Class B” uniform in the form of a green t-shirt with a custom logo on the front. This uniform can/should be used any time the Scout is going to be participating in an activity where they may soil or damage their official (and expensive) Cub Scout uniform.
